Surgical therapy of acending aorta aneurysm is ,usually performed trough midway sternotomy. This approach is considerably traumatic for the patient. The pain related to sternotomy can persist over time and in rare instance can cause severe problem to the patients. Recently minor invasive technique was introduced in cardiac surgery with the aim to develop for each operation a less traumatic approach. Today is possible to perform ,in selected circumstances (myocardial revascularization),operations through minimal incision and without extracorporeal circulation (5). Aortic valve replacement is performed through s or l partial sternotomy (6). Our preferred approach is l incision (fig.1). Our purpose is to present two cases of modified Bentall replacement of the ascending aorta performed through l-sternotomy showing the feasibility of complex operations with mini invasive approach.
